Policies and Procedures:
1. Please keep in mind that this is a request, pending approval.
2. All requests must be submitted at least one month in advance of the requested service date.
3. Once the date has been confirmed on the Church Calendar and the Staff Pastor presiding over the
event has been contacted, the request will be submitted to the Lead Pastors’ Assistant for final
4. Approval for all Service Involvement Requests will be up to the discretion of the Lead Pastors.
5. You will receive notification upon approval of your request in the form of this form being returned to
you with all necessary signatures. Copies of the approved request will be given to Seth & Caleb Perry, Amy Everett, Michael Sanchez, Pastors Adam & Liz Panepinto, the Staff Pastor presiding and Pastors Randy & Suzann Feldschau.
